Case Management Support

Description

PASAN provides individual support, information, and referrals to currently and formerly incarcerated people.

Individual Support Services include:


• Individual support and counseling
• Case Management: we work with clients to create a circle of care with community health care and service providers. This includes developing pre- and post- re-integration plans to connect clients to food, shelter, mental health supports, and primary health care services upon release
• Advocacy: we advocate on behalf of clients with prison health and community-based health providers to improve continuity of care and minimize treatment interruptions
